2. Make check payable to IAAP for $105.00 ($15.00 one-time processing fee, $58.00 IAAP dues, $12.00 Arkansas/Oklahoma division dues, $20.00 Central Arkansas chapter dues). Other payment options are indicated on the form.

3. Please include the following information under "Type of Membership" when completing this form. Professional is the standard classification of membership:

Division is A-O, # is 552000
